Aged & Disabled Waiver Case Manager - Allen and Surrounding Counties Classification:
Full-Time, 40 hour work week Supervisory

Summary The Case Manager's primary focus is to address the specialized case management needs of participants in the Aged and Disabled (A&D) Waiver program.
The Case Manager works in coordination with partner agencies to address the broad array of client needs through referrals to providers of necessary services.
The Case Manager acts primarily as a care coordinator and liaison, and may perform duties that include outreach, prevention and risk reduction, crisis intervention, and health education.
As part of the Case Manager's routine duties, he/she will interact with other governmental, faith-based, and nonprofit service providers for client services.
Responsibilities:
1.
Follow a person-centered process to assist clients with obtaining resources, service plan development and monitoring, and Medicaid recertification.
2.
Conduct face-to-face contacts with the individual (and family members, as appropriate) at least once every 90 days in the home of the waiver participant and as needed to ensure health and welfare and to address any reported problems or concerns.
3.
Compile case notes for each encounter with the participant.
4.
Establish and maintain collaborative working relationships with community resource providers.
5.
Provide relevant referrals to agencies and service providers based upon the goals and objectives mutually agreed upon with the client.
6.
Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services.
7.
Follow up with referrals to ensure that clients receive and access necessary services in a timely manner.
8.
Network with other agencies, coalitions, and the local community.
9.
Monitor clients' health and welfare.
10.
Monitor clients' satisfaction and service outcomes.
11.
Maintain files in accordance with State standards.
12.
Cultivate and strengthen informal supports for each participant.
13.
Identify resources and negotiate the best solutions to meet client needs.
14.
Actively participate in staff meetings and training.
15.
Perform other duties as assigned.
Minimum Qualifications (Must Have at Least 1 of the Following):
1.
A bachelor's degree in social work, psychology, sociology, counseling, gerontology, nursing, or health and human services.
2.
You may have a bachelor's degree in any field with a minimum of 2 years of full-time, direct-service experience with the elderly or disabled.
3.
A master's degree in a related field may substitute for the required experience.
4.
You must have a valid operator's license and state minimum liability insurance.
Physical Demands Case Managers typically spend time sitting, standing, climbing stairs, walking, driving, carrying (20lbs), lifting (20lbs), operating a computer keyboard, repetitive hand control, and operating a telephone.
